Replacement Eyeglasses Offer Available to Eligible ICS Students
To the Parents/Guardians of Irvington Community School Students:
If your child has either lost or broken his/her eyeglasses within the last year, you may be eligible to receive – at no cost to you or your family – replacement glasses. These replacements are being provided by your Learning Well school-based clinic through the generosity of Peyton Manning’s PeyBack Foundation, LensCrafters, Prevent Blindness Indiana’s Eye See Again program, and Irvington Community School. Due to the Christmas holiday, the deadline for the program has been extended to January 31, 2008. Please note: replacement glasses will be provided on a first-come, first-serve basis!
To be eligible:
1. The student must be 18 years of age or younger.
2. The student must have lost or broken his/her glasses after January 1, 2007.
3. You must contact the Learning Well clinic nurse before January 31, 2008, to tell her that your child may be eligible.
To get replacement glasses for your child:
1. As parent/guardian, you must call the Learning Well school nurse, Brooke Robeson, RN, BSN, at 357-5359, to tell her that your child may be eligible for the replacement glasses and to arrange to obtain the replacement glasses application.
2. The student must have a prescription from his/her eye doctor. You must bring a copy of this prescription to the Learning Well nurse, in addition to the completed application, to be eligible to get the voucher for the replacement glasses.
3. You will receive a voucher from Prevent Blindness Indiana that can be taken to any of the following LensCrafter locations: Glendale Mall, Greenwood Park Mall, Washington Square Plaza, and 4020 Lafayette Road.
